Attendance Policy

Thank you for trusting Family Achievement Center with your therapy needs. Our mission is to provide hope and improve lives through therapeutic excellence. You and your therapist have mutually agreed on the most beneficial frequency of therapy. While we believe that every scheduled appointment is important for success, we realize that is not always possible. At a minimum, a 75% attendance rate is needed to reach maximum benefit.

Cancellations, no shows, and tardiness compromise progress in therapy. We realize that situations arise, and we are happy to work proactively in managing those situations. Below we outline steps that need to be taken should you run into challenges with attending therapy. Please note, inability to comply with our guidelines will result in losing you/your child’s ongoing appointment times and/or being placed on a “same day schedule.”*

If the need arises to cancel an appointment, we request a 24-hour notice when possible as well as an attempt to reschedule. The following are general guidelines regarding absences, assuming a 75% or above overall attendance rate:

  • For absences up to 2 consecutive weeks, we recommend rescheduling one or both appointments (per therapy discipline).
  • For absences up to 3 consecutive weeks, you need to reschedule at least 1 week worth of appointments to maintain your current scheduled appointment times or you will be placed on a “same day schedule.”
  • For absences due to a medical issue (hospitalization, surgery, etc.), please notify the clinic as soon as possible to cancel the scheduled appointments. Your/your child’s scheduled appointment time will be held up to 3 weeks.
  • For absences more than 4 consecutive weeks, your ongoing scheduled appointments will be removed from the schedule.
  • For cancellations by the therapist, we will do our best to reschedule the patient with another provider and this will not be counted as a cancelled appointment or towards your attendance rate.

Tardiness:
We ask that patients are on time for all appointments. Please call the clinic if you are going to be late. If the patient arrives 15 minutes late or more for the scheduled appointment time, the therapist reserves the right to reschedule the entire treatment session, or it will be cancelled and considered a “no-show” (see below). If a patient is late 2 or more times in a 90-day period, loss of your/your child’s scheduled appointment time may occur.

Pick-up Times:
If you choose not to stay on clinic premises during a patient’s appointment, we ask that you arrive at least 10 minutes before the end of the appointment. If there are 2 or more late pick up times in a 90-day period, the caregiver will be required to remain on site during the patient’s treatment.

No shows/Cancels:
If a patient does not show up for their scheduled appointment without notifying the clinic ahead of time, the patient/caregiver will receive a phone call and will be asked to re-schedule. If you need to cancel, we ask that you attempt to reschedule your appointment to another time. Patients are expected to maintain a 75% attendance rate and failure to do so will result in ongoing appointments being removed and being placed on a same day schedule.

*Same Day Schedule:
Same day schedule means that the patient does not have a set, consistent day/time but could receive treatment by calling each week to see if an appointment is available. Same day scheduling is available as an option while waiting for a consistent ongoing appointment. Patients will be placed on a same day schedule if failure to comply with the attendance policy as no-shows, excessive cancellations, and tardiness impact patient progress.

Illness Policy

While regular attendance at therapy sessions is crucial for your child’s progress, we understand that patients get sick. We ask that you adhere to the following guidelines regarding illness. Please reschedule your appointment after the illness has been treated and resolved to maintain a 75% attendance rate.

  • If patient has a fever of 100.4°F or more, stay home for 24 hours after the temperature returns to normal without the use of fever reducing medication.
  • If patient has vomited one or more times due to illness keep them home until 24 hours after the last episode.
  • If patient has diarrhea that is abnormal compared with his/her normal pattern keep them home until symptoms subside.
  • If patient has a rash that the cause is unknown, check with a health care provider before sending them to therapy.
  • If patient has an illness that prevents them from participating in routine activities, do not attend therapy.
  • For head lice, we follow the American Academy of Pediatrics and patient can return to therapy following treatment of head lice.
  • If patient has eye drainage from a contagious conjunctivitis or other contagious illness, such as strep throat, keep them home for 24 hours after treatment is started.
