How do I schedule an evaluation?

To schedule an evaluation please call FAC at 651-738-9888. Prior to visiting FAC, we will need a referral from your doctor for your child’s therapy need (OT, PT, ST). Contact your child’s doctor to determine if they need to be seen for an appointment prior to coming to FAC for an evaluation. The referral from the doctor can be faxed to 651-738-9889.
